  • Assist primary care providers in recognizing, treating, and managing mental health and psychosocial issues as a key member of the primary care team.

  • Conduct comprehensive patient intakes focused on diagnostic and functional evaluations, making informed recommendations regarding treatment goals and plans.

  • Provide brief, focused interventions for patients requiring mental health support, implementing evidence-based best practices.

  • Advise healthcare providers on patient needs to determine when referral to specialized mental health resources is appropriate.

  • Initiate follow-ups to evaluate patient progress and propose adjustments to treatment plans as necessary.

  • Develop relapse prevention strategies and assist patients in maintaining stable mental functioning.

  • Identify at-risk patients and devise proactive plans to avert deterioration of their conditions.

  • Actively monitor and coordinate behavioral health care services, linking patients to additional treatment resources where needed.

  • Document clinical progress and diagnostics thoroughly in treatment charts to maintain comprehensive patient records.

  • Engage collaboratively with primary care providers to develop collective treatment plans that cater to patient needs.

  • Equip patients with self-management skills and information to empower their engagement in their treatment and recovery processes.

  • Provide consultations to clinic management and team members regarding behavioral services and potential program improvements.

  • Assist patients in complying with medical treatments prescribed by primary care providers, offering coping strategies for medication side effects.

  • Adhere to the Code of Ethical Conduct for Clinical Professionals and complete other duties as assigned.

Qualifications\n

  • A minimum of a Master’s Degree in Clinical Counseling.

  • Current LPC (Licensed Professional Counselor) license in Texas.

Compensation and Benefits\n
